A 26-year-old man seeking treatment for infertility was diagnosed with an exceptionally rare congenital condition after doctors discovered a uterus and structures resembling fallopian tubes inside his body. The unusual case of Persistent Mullerian Duct Syndrome (PMDS) was uncovered during an evaluation for primary infertility and azoospermia, a condition in which no sperm is found in semen.
New Delhi, September 10: A 26-year-old man seeking treatment for infertility was diagnosed with an exceptionally rare congenital condition after doctors discovered a uterus and structures resembling fallopian tubes inside his body. The unusual case of Persistent Mullerian Duct Syndrome (PMDS) was uncovered during an evaluation for primary infertility and azoospermia, a condition in which no sperm is found in semen.
Doctors at RG Hospitals, Rajouri Garden, said the patient had a 46,XY chromosome pattern and both testes had remained undescended inside the abdomen. An MRI scan revealed a uterus-like structure along with tubular structures resembling fallopian tubes, prompting further investigations.
PMDS is a rare condition in which Mullerian structures, including the uterus and fallopian tubes, persist in a person with typically male chromosomes and male physical development. Fewer than 300 cases have been reported in medical literature, and the condition is generally identified during childhood, often while investigating an undescended testis. The patient underwent laparoscopic surgery to remove the abnormal Mullerian structures and both severely damaged testes, NDTV reported. The surgery was performed by Dr Susheel Kharbanda, Chief Urologist at RG Hospitals.
However, examination of the removed tissue revealed another significant finding. The left testis showed Germ Cell Neoplasia In Situ (GCNIS), a pre-cancerous condition that can potentially progress to testicular cancer. The right testis showed severe atrophy but no GCNIS. Doctors said the case highlights why unexplained infertility, particularly when accompanied by bilateral undescended testes, may require detailed imaging, genetic testing and tissue examination. The case also underscores that normal tumour-marker blood tests do not necessarily rule out early or pre-cancerous changes.
