Delhi: Man Discovers Uterus During Infertility Tests, Diagnosed With Rare PMDS Condition
A routine fertility checkup in Delhi led to an unexpected medical finding after a young man undergoing evaluations for primary infertility was diagnosed with Persistent Müllerian Duct Syndrome (PMDS)—a rare congenital condition where female reproductive structures develop in a biologically male individual.
A 26-year-old man undergoing infertility evaluation in Delhi was found to have a uterus and fallopian tube-like structures, leading doctors to diagnose him with Persistent Müllerian Duct Syndrome (PMDS), a rare congenital condition, according to a report. The case was further complicated by severely damaged undescended testes, one of which showed pre-cancerous changes, doctors said.
The man had sought medical help after facing infertility issues and was found to have azoospermia, a condition in which no sperm is detected in semen. MRI Reveals Uterus-Like Structure
Doctors conducted imaging tests, including an MRI, which revealed an underdeveloped uterus and structures resembling fallopian tubes. Genetic testing reportedly showed a 46,XY chromosome pattern.
The findings led doctors to diagnose Persistent Müllerian Duct Syndrome, a rare condition in which Müllerian structures, which normally develop into female reproductive organs, remain present in an individual with male chromosomes and typically male external characteristics. Why PMDS Occurs
During typical male fetal development, anti-Müllerian hormone helps cause the regression of Müllerian ducts. PMDS can occur when there is a problem with the production or action of this hormone.
The condition can remain undetected for years because affected individuals may have typically male external genitalia. It is often associated with undescended testes and may be discovered during investigations for infertility or other medical conditions.
Doctors Perform Laparoscopic Surgery
Doctors at a Delhi hospital performed laparoscopic surgery to examine and treat the condition. During the procedure, they confirmed the presence of the underdeveloped uterus and other Müllerian structures, along with both testes located inside the abdomen.
The Müllerian structures and severely damaged testes were subsequently removed after doctors assessed the patient's condition and the potential health risks.
Pre-Cancerous Changes Found in One Testis
A microscopic examination of the removed tissue found severe atrophy in both testes. One testis showed Germ Cell Neoplasia In Situ (GCNIS), a pre-cancerous condition that can potentially develop into testicular cancer.
The case highlights the importance of detailed evaluation in patients with infertility and undescended testes, as certain rare congenital conditions may remain undiagnosed until adulthood.
